MIRAMAR, Fla., Nov. 5, 2026 /PRNewswire/ — Integrated Home Care Services (“Integrated”) and CareCentrix today announced an agreement to combine, subject to required regulatory approval. The proposed combination advances a shared vision for a comprehensive, independent in-home benefit management platform and would bring together complementary geographic reach, deep clinical and operational expertise, and capabilities across a broad range of in-home services for all payer member populations.

Integrated Home Care Services and CareCentrix Announce Agreement to Combine, Advancing Comprehensive In-Home Benefit Management Platform

As healthcare increasingly shifts into the home, health plans face growing pressure to improve affordability, quality, and member experience while reducing internal administrative costs. Care at home remains fragmented, often requiring members, families, and care teams to navigate multiple providers, services, and disconnected processes due to traditional approaches. Integrated and CareCentrix share a commitment to simplifying this experience and improving affordability through specialized expertise, high performing provider networks, technology-enabled benefit management and value-based alignment.

The proposed combination would bring together five defining capabilities in one comprehensive nationwide platform:

  • Complete payer population coverage with proven results: Supporting Medicare Advantage, Managed Medicaid, Commercial, ACA, and other risk-bearing and self-insured populations.
  • Multi-service benefit management: Coordinating benefits across home health, personal care, long-term services and support, durable medical equipment, home infusion, private duty nursing, sleep-related services, and other post-acute transitions of care services from hospital to home.
  • Flexible service models: Enabling health plans to choose from technology-enabled care coordination and referral management to fully delegated benefit management or tailored hybrid solutions.
  • National capabilities across all members and major in-home services: Ready to provide in-home benefit management or technology enablement in all 50 states plus Puerto Rico, supporting the largest national payers to emerging local health plans.
  • Flexible value-based contracting and risk models: Offering tailored financial structures including capitation, shared savings, and fee for service with compelling performance guarantees – backed by decades of experience across all member populations.

Together, these capabilities are expected to simplify benefit administration, improve access to timely, high-quality care, enhance the member and provider experience, improve affordability of in-home care services and reduce avoidable hospitalizations and emergency department visits. The shared vision is to make care at home easy, affordable, and integrated.

“Health plans need more than a collection of vendors to realize the full potential of care in the home,” said Christopher Bradbury, CEO and President of Integrated. “They need a trusted, specialized partner that understands their members, their business, and the complexity of delivering care at home. Our vision is to bring together the reach of a national platform across all member populations, the expertise of a specialist, and the flexibility to meet health plans wherever they are in their care-at-home strategy. Together, we see an opportunity to make care at home easier, more affordable, and better integrated for everyone. The combination accelerates this vision.”

“Members often need multiple types of care and support at home, yet navigating those services can be unnecessarily complex,” said Haley Sofiane, President of CareCentrix. “By bringing together our complementary capabilities, we see an opportunity to simplify that experience, strengthen coordination across services, and help health plans deliver better care and greater value to the people they serve.”

Upon closing, Christopher Bradbury will serve as Chief Executive Officer and a member of the board of the combined organization. Haley Sofiane will serve as President and Chief Operating Officer. The combined organization will be headquartered in Miramar, Florida.

The proposed combination is expected to close in the first half of 2027, subject to required regulatory approvals and customary closing conditions. Until closing, Integrated and CareCentrix will continue to operate as separate, independent organizations.
